A Kidz Clinic offers a sliding fee scale regardless of ability to pay, race, ethnicity, disability, or employment status.
A Kidz Clinic offers a discount based off of household income and family size. Sliding fee calculations are determined by using federal income tax information, W-2’s, or two consecutive pay stubs. We will use the most up to date sliding fee scale table to determine eligibility status. We may assist you with a Medicaid application if you do not have this letter from Medicaid.
If you do not have any source of income, we will review your banking statements.
